By State of Illinois law, generally all new students are required to present proof of immunity from:
- Diptheria
- Tetanus
- Pertussis
- Rubella (German measles)
- Measles
- Mumps
- Meningococcal disease (Meningitis A) for students under 22 years beginning in the fall of 2016
The University notifies all new students of the immunization requirement and provides forms and instructions for compliance. Information and forms are also available on the UChicago Student Wellness website.
After the fourth Friday of the first quarter of registration, students who are not yet compliant will have their subsequent registrations restricted and will not have the restriction lifted until they have become compliant with the immunization requirement. Students who receive this notification are urged to send a message to the Immunization Team through the my.WellnessPortal message center to resolve their immunization compliance status.
